COMPLICACIONES DURANTE LA LIMPIEZA Y CONFORMACIÓN DE LOS CONDUCTOS RADICULARES Gutmann 155

            Fractura de instrumentos dentro del conducto radicular.

Causas:

La causa más común es el uso inadecuado de los instrumentos radiculares.

Prevención:

A. Los instrumentos deben ser desechados y reemplazados cuando:

1. Se detecten fallas como áreas brillantes o como aplastamientos en los filos.
2. El uso excesivo ha causado dobleces excesivos o arrugamientos.
3. Ha sido necesario una curvatura excesiva.
4. Se ha producido una curvatura accidental durante el uso.
5. La lima se anguló  en lugar de curvarse.
6. Se nota corrosión en el instrumento.
7. Los instrumentos para condensación tienen puntas defectuo­sas o han sido excesivamente calentados.

B. Usar abundante irrigación
C. Ejercer limado circunferencial corto 1 a 3 mm de entrada - salida de la lima, sin rotarla en ningún sentido.
D. Usar limas hedstrom sólo para terminar las paredes del conducto, una vez ampliado.
E. No avanzar demasiado rápido en el cambio de instrumentos o saltárselos en su numeración.

Solución

1. Uso de excavadores, exploradores.
2. Uso de limas hedstrom. Más útiles para remover puntas de plata que para instrumentos separados.
3. Pinzas especializadas. Steiglitz, fórceps Peet Splinter, etc.
4. Estuche de instrumentos Masserann (erróneamente denominado Massermann en muchos artículos). Fabricado por MicroMega, Francia, contiene una serie de trépanos y dos series de extractores tubulares. Su uso consiste en que primero se talla un espacio alrededor del instrumen­to de unos 2 mm en su extremo coronal para que el extractor pueda atraparlo y removerlo.

Masserann instrumentation   The well established MASSERANN technique for the removal of metal fragments (posts, burs etc.) from root canals is based on the principle of freeing the obstruction around its periphery. One of its many advantages is that the fragment is removed quickly with minimal loss of substance and without the danger of pushing it further into the canal.

1. 1. Open access Enlarge canal orifice using round bur. Use Gates drill to create straight access to the fragment.
   2. Clear fragment Use trepan bur to set free approximately 4 mm of fragment (slow anticlockwise rotation). Masserann trepan burs can be used either by hand with the short manual handle or with a reducing contra-angle (300-600 rpm) with attachment system according to ISO 1797-2 standard (tightly screw with the flat spanner).
   3. Grip & remove fragment • Slide opened extractor over freed end of fragment. • Hold extractor firmly in place and turn the screw head until it fragment   is gripped. • Remove fragment : -in anticlockwise rotation for all cutting root canal instruments. -in clockwise rotation for all filling instruments (ex : spiral fillers). Bear in mind that... • Fragment must always be removed in rotation – never pull out only. • Direct straight access is a must: Masserann tools are straight. • Positioning the instruments is most important: - Use your finger to guide the trepan in the right direction. - 2 Xrays from different angles will help checking orientation of fragment before and when applying extractor. Cleaning: - Immerge in a detergent bath, possibly ultrasonic cleaning bath. - Cleaning, disinfection and sterilisation instructions for root canal instruments apply to the Masserann instruments. Sterilization : - Autoclave - 135°C/2.1 bars (275°F/29psi) – 18 mn minimum under sterilization bag.

Remoción ultrasónica. El aparato ENAC (Osada Electric Co.) tiene suficiente energía para recuperar fragmentos metálicos. Si se usa en combinación con el extractor de Masserann puede ser más efectivo.
